A case report form (CRF) is a printed, optical, or electronic document designed to record all protocol-required information on each subject in a clinical research study. The CRF facilitates complete and standardized data collection that promotes efficient processing, analysis, and reporting of information, as well as exchange of data across sites and to the Sponsor/Principal Investigator/Data Coordinating Center.

Editing Form Instructions

All posted case report forms are protected documents to allow only filling in forms. In order to edit existing content, the form must be unprotected first, please follow the instructions below to unprotect/reprotect the form.

Unprotect/Protect Form

Word 2007 - 2010

  1. Open protected form
  2. Click the "Review" tab on the ribbon
  3. Click the "Protect Document" button
  4. Click "Restrict Formatting and Editing"
  5. Click "Stop Protection" button on the task panel
  6. Click " Yes, Start Enforcing Protection" button on the task panel to reprotect the form

Word 2000 - 2003

  1. Open protected form
  2. Click the "Tools" menu
  3. Click "Unprotect Document"
  4. Click "Ok"
  5. Repeat step 2 and 3, Click" OK" button to reprotect the form
