Template Letter – Students

Statement of Explanation and Management (SEAM)	If you disclose a significant financial conflict of interest on a Proposal Approval Form (PAF) (e.g. funding application) or an IRB application (e.g. human subjects approval), then you must complete the SEAM form.	Guidelines
Statement of Explanation and Management (SEAM) – Familial COI NEW!	If you disclose a familial conflict of interest on a Proposal Approval Form (PAF) (e.g. funding application), then you must complete the SEAM form for Familial COI.
Statement of Explanation and Management (SEAM) – Institutional COI NEW!	If you disclose an institutional conflict of interest on an IRB application (e.g. human subjects approval), then you must complete the SEAM form for Institutional COI.
