Summary of Changes to UIC ACC Protocol Forms*:

  1. Form A; Version 3.3:- Changes made in items 8a, 8c, 9c2, 10e, and 11:

    1. Ordering of animals (8a): [For purchasing rodents and rabbits: Should investigators require rodents or rabbits from a source other than a BRL approved vendor, they should contact BRL veterinary staff at (312-996-7040) to discuss health status and/or quarantine requirements.]
    2. All locations of use with procedures (8c): [Listing of all the Locations in which animals will be used and the procedure(s) that will be conducted in each location (e.g., surgery, euthanasia, behavior testing, etc) is required.]
    3. Scientific justification of use of ether (9c2): [if ether is being used as anesthetic agent, then a scientific justification for its use must be provided.]

  2. Appendix 3; Version 2.2 and Form D; Version 2.3, item 4h:

    1. Addition of statement regarding protocols: [The following statement should be acknowledged and agreed to by PI; A copy of the final, approved version of this protocol and any subsequently approved modifications involving the use of animals will be available to all personnel listed on this appendix and the principal investigator ensures that all personnel are familiar with the protocol.]

*Although ACC strongly advises the PIs to use the new version of forms listed above, for the PIs who wish to submit the previous version of forms (Form A; version 3.2, Form D; version 2.2 and Appendix 3; version 2.1), please make sure all the changes mentioned above are addressed in your protocol at appropriate sections. No version older than these will be accepted.
