OACIB Holiday Schedule 2009

The Office of Animal Care and Institutional Biosafety (OACIB) serving the Animal Care and Institutional Biosafety Committees (ACC/IBC) will observe the UIC reduced service schedule for non-essential services beginning on Thursday, December 24th. Therefore, OACIB will close at 5:00 PM on Wednesday, December 23, 2009, and will reopen at 8:30 AM on Monday, January 4, 2010.

Please remember that the deadline for submission for protocols eligible for review at the January Meetings is:

  • 5:00 PM, Tuesday, December 15, 2009 for ACC protocols and
  • 5:00 PM, Wednesday, December 16, 2009 for IBC protocols.
In addition, to allow time for the review and approval process prior to the reduced service schedule, modifications eligible for administrative or designated review must be submitted by 9 AM on Thursday, December 17, 2009.

To download a form:

  1. Right-click on the format of the form you want to download (e.g., Word, PDF, etc.)
  2. Depending on your browser, select Save Target As or Save Link As.
  3. Save the file somewhere on your computer where you will remember where to find it (e.g., C:\My Documents\invention_disclosure.doc).
  4. Then, open the file from your computer.

To complete a form:

Use the table below to determine which forms to complete. Then, use Chapter 3 of the Animal Users Manual: Protocol for Animal Use, Completion and Review and the UIC ACC policies and guidelines to complete the forms.

Rules

  • For all research protocols, completion of Appendix 3 for personnel qualifications is required. All personnel must have completed mandatory ACC Training prior to approval of a protocol on which they are listed or prior to their addition to a protocol via modification.
  • For all research involving animal breeding, appropriate Appendix 1 forms must be completed.
  • For all research involving the use of hazardous materials in animals, Appendix 2 must be completed. (e.g., if you answered "YES" to any item listed in Form A, item 9a)
  • For all research sponsored by multiple funding sources, completion of Appendix 4 is required.
  • For all research involving rDNA, infectious agents/toxins in animals, appropriate IBC forms must be completed and IBC approval must be obtained prior to initiation of the work.

IF your protocol submission involves…
THEN complete…
euthanasia only, Form A.
no potential pain, Form A.
only minimal or transient pain, Form A and Form B through item 4.
potential pain or distress with or without pain control, Form A and Form B.
a purpose that is primarily for teaching or training, Form A, Form B as appropriate & Form C.
modifications to an approved protocol, Form D.
multiple ACC protocols associated with a single grant (RO1, program project grants, center grants, etc.) - See Note Below

Form G

institutional training or travel grant - See Note Below Form G
subcontract of any animal work to a non-UIC performance site - See Note Below

Form G

establishment of transgenic/knockout animals, Appendix 1a.
establishment of germline transmission, Appendix 1b.
maintenance of a breeding colony, Appendix 1c.
use of hazardous materials in animals, Appendix 2.
any personnel working with or handling animals, Appendix 3.
multiple grants, Appendix 4.

NOTE:  Submission of Form G does not substitute for or waive the requirement for submission of other ACC protocol for animal use forms for work conducted at UIC.  See When to Use Form G for examples and what documentation is required.
