Important: Old ACC Forms No Longer Accepted
This is a reminder that as of July 1, 2004 only current versions of the ACC forms will be accepted for review and approval.Beginning July 1, new protocol applications submitted on old forms will be returned to investigator without review. Protocol applications that were submitted on version 2 forms prior to July 1, will still be reviewed.
All new forms are available on the ACC web site. Acceptable version numbers are listed below:
